The way iTunes and iOS 6 handle podcasts seems clunky and is likely a factor. Even after this, iTunes thinks there is 8 GB free but the device says 4 GB, so there's a disconnect there. These programs group dupes so you can keep. To really, really get rid of the image, Control-click (Right-click) on the Trash entry that appears at the bottom of iPhoto’s Source list and choose Empty Trash.
